Donor Family Quilt

The LifeSource Donor Family Quilt, established in 2004, is a patchwork of memories, lovingly created by donor family members in our region. If you would like to contribute a memory patch in honor of your loved one, please contact one of our donor family advocates or follow these guidelines.

Although you’ll need to mail your quilt square and information about your loved one to us, please feel free to email your tribute to familyservices@life-source.org. The quilt is an ongoing project; there is no timeline for submitting a square.
