Upper Midwest Specialty License Plates

South Dakota
Within our service area, South Dakotans have the unique opportunity to purchase Donate Life license plates at their motor vehicle office.
After purchasing the plate, you can request a decal using the form below. These are provided at no cost and will be mailed to you upon receiving your request. The decal can be placed on both the front and back license plate in the designated area.
Minnesota & North Dakota
Legislation is in-progress for Minnesota to activate a Donate Life specialty license plate, a service that may become available at your local Department of Motor Vehicles (DMV) office. Continue to check back for updates!
LifeSource offers Donate Life vinyl decals available upon request that can be on your vehicle. These decals will be provided at no cost. Fill out the form below to request your decal.

Click here to visit Donate Life America’s Specialty License Plate list. Each state has specific requirements to meet in order to offer a specialty license plate. If you are interested in having a Donate Life specialty plate in your state, contact your local representative. They may already have an application process or petition started in order to achieve this goal.
