How to Update Your Registration Status

Everyone can register as a donor – it’s a decision that saves lives. However, if you change your mind about your decision, you can change your registration too.

When you register as an organ, eye and tissue donor, you are making a legal decision and, even after your death, your decision will be honored. It’s important to talk with your family so they are prepared to honor your decision at the time of your death.

How to Register as a Donor

There are many ways you can register as a donor: at the DMV, online and with your phone.

1. Check the box on your driver’s license

Register to be a donor on your state ID card or driver’s license. Simply check the box on your application when renewing or applying.

3. In Your iPhone Health App

Open your iPhone’s Health app. Tap on your image or initials at the top right to access your profile. Scroll down until you see “Features” and/or “Organ Donation.” Tap Sign up with Donate Life.

To later change your donor information or remove your registration, tap your profile picture or initials, tap  Organ Donation, then tap Edit Donor Registration.

4. When you apply for your hunting and fishing license (Minnesota-only)

Minnesotans also have the opportunity to register as a donor when applying for their hunting and fishing license online. Get your hunting and fishing license here!

No matter how you register, the last step is to share your wishes about being a donor with your family. Your family members will be the ones to honor your wishes after you’re going, so please be sure to tell them how you feel.

How to Unregister as a Donor

If you registered as a donor on your driver’s license or state ID card

You will need to visit your local DMV to remove the status from your driver’s license. You will need to request a duplicate license and write the word “REMOVE” on the application where you indicate your donation decision. Please note, there is a charge for a duplicate license.

Minnesota Online: If you registered at DonateLifeMN.org or Life-Source.org

Please contact LifeSource at info@life-source.org with the following information to verify and request removing registration:

  • Legal first and last name
  • Date of birth
  • Gender

You will receive an email back reply back from info@life-source.org once confirmed and processed.

North Dakota Online: If you registered online at DonateLifeND.org or Life-Source.org

The North Dakota Department of Public Safety allows you to update your organ donor status online. You will need to provide your First Name, Last Name, Driver’s License or State ID card number, date of birth and the last four digitals of your social security number. Under donor designation, you will need to select one of the two options:

  • Please add my name to the North Dakota Donor Registry.
  • Please remove my name from the North Dakota Donor Registry.

These changes will be reflected on your next driver’s license and/or state ID card.

South Dakota Online: If you registered at DonateLifeSD.org or Life-Source.org

The South Dakota Department of Public Safety allows you to update your organ donor status online. You will need to provide your First Name, Last Name, Driver’s License or State ID card number, date of birth and the last four digitals of your social security number.

Under donor designation, you will need to select one of the two options:

  • Please add my name to the South Dakota Donor Registry
  • Please remove my name from the South Dakota Donor Registry

These changes will be reflected on your next driver’s license and/or state ID card.

Other States

If you registered in a state outside the LifeSource service area (MN, ND, SD) you can contact your local Organ Procurement Organization (OPO) to learn how to update your status in that state.

National Registry: If you registered online at RegisterMe.org

You can check your registration status at any time. Go to RegisterMe.org. Above the form, you will see “Already registered? Access registration.” Click on “Access Registration.” You will be required to provide:

  • First and last name
  • Date of birth
  • Zip code
  • Last four digits of your SSN
  • Driver’s License Number

Regardless of your decision, it’s important to have a conversation with your loved ones about organ, eye and tissue donation.
